If you see a crime/incident on campus and you would like to report it, please fill out as much as you can on the form below. We encourage you, if possible, to give us your contact information so we may follow up with you. ALL INFORMATION WILL BE KEPT CONFIDENTIAL.
This form will be submitted directly to University Police. If it is a crime, they will investigate and follow up as much as possible. If it is an incident, it will be forwarded to Judicial Affairs, the Office of Affirmative Action and the Title IX Coordinator. All of these offices will be working closely together on any issue submitted here. We would prefer you also add your contact information so that we might follow up with you. You do NOT have to leave this information, but it would be very helpful. This process is CONFIDENTIAL.
